Pet Registration Form

If your pet is up to date on their rabies vaccination, please continue to fill out this application for a pet license. If you have more than one pet, please complete a separate form for each pet.

Notes/Reminders:

1. Although fees can be lumped into one payment for multiple pets, please complete a separate form for each pet below.
2. Rabies Vaccinations for new or expired rabies tags can be emailed to the office at villagehall@edgarwi.org or stop in at the office

3. Once rabies vaccination and fees are received the license for your pet(s) can be mailed to you at your home.

4. Remember, that if the rabies vaccination for your pet expires during the licensing year, you are required to bring in or send in a copy of their new vaccination record.
